3. Payment can be cash, check or credit card.

Credit card payment can be taken over the phone (360) 491-4728.

4. If FAXing Registration, payment must be made by Credit Card over the phone.

PLEASE DO NOT INCLUDE CREDIT CARD NUMBERS ON ANY OF THE FORMS.

5. Up to 10 days prior to class start date (or on the 1^st day of class) your student can receive a DOL PERMIT WAIVER FORM. That form will allow them to get their Learner’s Permit without taking the Knowledge (written) Test at DOL.

THIS FORM MUST BE COMPLETED IN FULL AND SIGNED BY AN INSTRUCTOR BEFORE TAKING IT TO DOL!

6. What to bring to DOL to receive the Learner’s Permit.

Basically: Student’s Birth Certificate (original or certified copy) and/or

Passport. And

Social Security Card. And

School ASB Card or WA State ID Card.

MOST IMPORTANT: $20!!
